Here's a pic of the matrix on my Equinox-Labyrinth copy.
This copy comes from a second hand shop and I guess it has been sitting there for years. However would still like to have confirmation.

Here's a pic of the booklet. The left side shows advertisements for 7 lp's (Ruffians/Griffin/ Sodom/Assassin) and 2 cd's (Destruction/Satan).
